Dana Huete, MSN, RN

Executive Staff

Dana Huete is a native New Orleanian. She is married and has two sons.  Dana is a Registered Nurse with 20 years of experience in mostly the ambulatory care setting.  She spent most of her career at Tulane Medical Center as the Director of Ambulatory Nursing and Clinic Operations.  Dana also has a wealth of experience in Value Based Care, Clinical Integration, Accountable Care Organizations, Quality Management and Process Improvement.  Her most recent experience is working with Federally Qualified Health Centers, (FQHCs) managing their Value Based Care contracts, Population Health Initiatives, HEDIS Performance and clinic operations best practice.  Dana’s experience allows her to support providers, health centers and payors in our ever-changing healthcare arena.

Lori Meyers, BSN, RN

Executive Staff

Lori Meyers was born and raised in New Orleans, currently lives in Slidell, LA, is married and has one 20-year-old son. Lori is a Registered Nurse with 22 years of experience in a variety of nursing specialties such as critical care, quality management, hospital administration, process improvement, clinical integration, and Value Based Contracting. Her most recent experience is working with Federally Qualified Health Centers (FQHCs) negotiating/managing their Value Based Contracts, quality HEDIS performance, and clinic transformation. She has years of knowledge in developing processes that will improve quality and efficiencies for providers as well as payers. She has also acquired knowledge with integrating and validating multiple data sources; thereby, creating documents that are valuable to healthcare providers and large organizations that are managing cost and patient quality.  Lori Meyers’ experience allows her to support providers and payers in this shift from fee-for-service to pay-by-performance models that are evolving in healthcare today.

Patricia Chandler, BSN, RN


Patricia Chandler has been a nurse for over 25 years. She was raised in Slidell, Louisiana.  She received her BSN from the Lurleen B. Wallace School of Nursing at Jacksonville State University in Jacksonville, Alabama. She has been the recipient of several awards including the City Business of New Orleans Healthcare Hero award and recognized as a Great 100 Nurse in Louisiana. She has had a diverse career as an emergency room nurse, quality and performance improvement and risk management coordinator, Risk Manager and Stroke Coordinator. She has assisted with various IT application implementations and software builds. She has a vast knowledge in population health tools and data mining/integration.  Patricia also worked as a Clinical Integration Nursing Supervisor and currently serves as the Director of Clinical Informatics at Target Health.

Lee Reilly, RN

Director of Value Based Care Performance
Lee Reilly has been a nurse over 40 years. She lives in New Orleans, Louisiana, is married, and enjoys spending time with her grandchildren. Lee is board certified in case management and has over 20 years of managed care experience in Commercial, Medicaid, and Medicare Advantage health plans overseeing utilization management, case management, and quality management. Lee’s most recent experience includes value-based contracting, supporting providers in providing value-based care and understanding the transition to alternative payment models. Lee is Target Health’s Director of Value-based Care Performance. In this role she oversees their nurse call center where a team of nurses focus on telephonic patient engagement through both inbound and outbound calls. During the call activity, the nurses motivate patients to visit their primary care provider to receive preventive services, follow-up after emergency department and hospital care, and to manage their chronic health conditions.
